How to Find Your Form URL

Go to the forms section of Blitzen (accessible from the left panel) 

Single-click on the form name in the middle panel. 

Beneath the form name in the right panel you will see the form URL.

Right-click and select 'Copy Link Address'

Now you have the form URL!
